Job Summary*
We are looking for Community First Services and Supports (CFSS) Program Coordinators. This is a great position for a case manager seeking to empower people to use services in their own communities. A successful candidate will enjoy meeting new people; have excellent interpersonal skills; deliver detailed work, stay organized and meet deadlines; and have a strong desire to successfully deliver services within the required guidelines of state statutes.

- Develop and maintain strong relationships with clients, acting as their primary point of contact.

Education and Experience:
- Four-year degree in human services or related field with one year for direct care to individuals with disabilities or individuals of 65 years of age or older
- OR Two-year degree in human services or related field with two years of direct care to individuals with disabilities or individuals of 65 years of age or older.
- Demonstrated interpersonal skills that quickly build rapport with customers, county social workers, and administrative staff.
- Case management experience related to self-directed services preferred.
- Candidate must complete and pass a background study, have a valid driver’s license, proof of motor vehicle insurance and reliable transportation for work purposes.
Job Type: Full-time
Pay: $20.00 - $24.00 per hour
