Global Health Link provides 245D services, Housing stabilization services, PCA services, and Customized Living services to individuals that receive waiver benefits through Medical Assistance. We've been operating in the Twin Cities since 2007 and pride ourselves in providing quality services.
Job Summary
Housing Stabilization Coordinators will assist individuals with disabilities in locating and maintaining adequate and affordable housing. The Housing Stabilization Coordinator is responsible for identifying barriers to housing and assisting clients through the process of relocating to community-based housing. Housing Stabilization Coordinators provide the needed support and advocacy for our clients along the way.
In this role, you will begin by creating an individualized plan to identify barriers that have kept clients from finding long-term housing. Housing Stabilization Coordinators will continue working with clients after they've found housing. You will make a plan to help the client keep the home they've found. You will monitor the client's adherence to the plan throughout the year.
-Coordinators carry a caseload size of around 10-13 clients.
-Paid mileage rate of .575 cents/mile.
QUALIFICATIONS:
- Bachelor's Degree or equivalent experience
- Reliable vehicle/valid driver's license and ability to transport clients (Required)
- Good knowledge of the Twin Cities Area (Required)
- Knowledge of benefit/assistance programs in Minnesota (Required)
- Housing Access Coordinator Experience (highly preferred)
- Experience working with vulnerable adults (preferred)
- Housing or Human Services experience (preferred)
- Ability to quickly learn new software systems
Job Type: Full-time
Pay: $20.00 - $26.00 per hour
